The Health Center Program: Change in Scope

When to Request Change in Scope

Program regulations require that HRSA-funded health centers receive prior approval of revisions to the budget and project plan whenever there is a significant change in the scope or nature of project activities.

Request a change in scope when you:

  • Add a site
  • Delete a site that was previously approved by HRSA.
  • Relocate a site. When relocation affects the budget, services provided, numbers of patients served, or the number and types of providers, it requires prior approval.
  • Discontinue a service previously approved by HRSA.
  • Add a service.

Health centers must request approval for changes in scope at least 60 days before the planned date of implementation.

You do not need to request prior approval when you:

  • Add a service to a site that is already approved
  • Change the number of intermittent sites or voucher locations
  • Change providers
  • Change hours of operation

Changes in providers, hours of operation, service area and target population must, however, be documented in the health center's next application for Federal grant support.
