Service Sites
A service site is any place where a health center provides primary health care services to a geographic service area or population.
Service sites must meet the following conditions:
- Face-to-face encounters between patients and providers occur and are documented in the patient's medical record.
- Providers exercise independent judgment in the provision of services to the patient.
- Services are provided by or on behalf of the grantee.
- Services are are provided on a regularly scheduled basis.
Services
HRSA-funded health centers are required to provide a defined set of primary health care services. These may be related to
- family medicine
- internal medicine
- pediatrics
- obstetrics and gynecology
- diagnostic laboratory and radiological services
- pharmaceutical services as appropriate
- defined preventive health services
Once a service is included in the scope of project, it must be provided equally to all patients, regardless of their ability to pay.
Providers
Providers are health care professionals who deliver services to patients on behalf of the health center. They are responsible for assessing the patient and documenting services. All providers must be properly credentialed and licensed to perform activities expected of them by the health center.
Service Areas
A service area is the geographic area served by the health center, where the majority of its patients reside.
Target Population
The target population is the medically underserved community or special population to be potentially served by the health center.
